Once documents have been determined to be complete and are in order, the informant either submits the documents in person or sends it by mail to the Consulate, taking note of the envelope's tracking number. The death of a Filipino should be reported to and registered with the Philippine Statistics Authority (PSA) through the embassy/consulate general having jurisdiction over the locality where the event took place.

The Philippine Consulate General in New York covers the following states: Connecticut, Delaware, Maine, Massachusetts, New Hampshire, New Jersey, New York, Pennsylvania, Rhode Island, and Vermont. Mortuary certificates are issued when remains or the cremated remains of a deceased are to be transported to the Philippines.
