Arrange the four (4) sets of
Report of Death in this order; 
One (1) set should have all original documents.
Arrange the two (2) sets of
documents in this order; 
One (1) set should have all original documents.
Arrange the four (4) sets of
documents in this order;
 One (1) set should have all original documents.

 


 

The envelope containing the required documents should be addressed to:

Note: Ensure that all requirements have been complied with. Otherwise, incomplete submissions will not be processed and will be returned to the informant at their own cost.

 

Above documents should be received by the Philippine Consulate General AT LEAST SEVEN (7) WORKING DAYS PRIOR TO DEPARTURE DATE.  The Philippine Consulate General assumes no responsibility for any delay or loss in the mail, or while the documents are in the custody of the courier service. The Consular Officer reserves the right to require additional documents from the informant/applicant.  The informant/applicant should note the tracking numbers of all envelopes used and submitted, and may track their envelopes at the website of their chosen courier.

It takes around eight (8) months to one (1) year from the date of filing of Report of Death before the PSA can generate a copy of the requested document on security paper with a barcode. Once available, you may request online for a hard copy of the report of Birth/Marriage/Death at http://www.psaserbilis.com.ph.
