PSA-issued Birth Certificate (Mandatory)\u00a0<\/strong><\/h4><p>One original and one (1) photocopy of the Birth Certificate issued by the Philippine Statistics Authority (PSA or formerly National Statistics Office\/NSO).\u00a0<\/p><p><em><span style=\"font-size: 10pt;\">If you do not have a PSA-authenticated birth certificate, you can order one online through <a href=\"http:\/\/www.psaserbilis.com.ph\" target=\"_blank\" rel=\"noopener\"><b>http:\/\/www.psaserbilis.com.ph<\/b><\/a> OR the applicant may wish to designate a representative to secure it from the PSA and send it via courier.<\/span><\/em><\/p><p><em><span style=\"font-size: 10pt;\">If there is no record of birth with the PSA, check first with the local civil registrar (LCR) where the birth took place.\u00a0 If LCR birth record is available, the LCR should forward\/endorse its copy to the PSA.\u00a0 If LCR does not have a copy of applicant\u2019s birth record, the applicant or his\/her duly appointed representative may apply for late registration of birth at the local civil registrar at the place of birth.<\/span><\/em><\/p><p><span style=\"font-size: 10pt;\"><em>The applicant should request for a PSA-authenticated birth certificate once the LCR have endorsed the applicant&#8217;s birth record to the PSA.<\/em><\/span><\/p><h4><strong>C.
