A. Core requirements for Principal Applicant:<\/strong><\/span><\/p>

  1. One (1) duly accomplished Dual Citizenship Application Form <\/span>(click here)<\/span><\/a><\/span><\/li>
  2. Philippine Statistics Authority (PSA)-issued Birth Certificate (<\/span>original + 1 photocopy<\/b>).<\/span><\/span><\/li>
  3. Latest Philippine Passport (<\/span>original + 1 photocopy of data page<\/b>).\u00a0<\/span><\/span>
    1. If the applicant is not able to present it, the Consulate requires a notarized affidavit of loss.<\/span><\/li><\/ol><\/li>
    2. For married women applicants: PSA Marriage Certificate \/ Report of Marriage (ROM) \/ US or Foreign Marriage Certificate (<\/span>original + 1 photocopy<\/b>).\u00a0<\/span><\/span><\/li>
    3. For widows: Death Certificate (<\/span>original + 1 photocopy<\/b>).\u00a0<\/span><\/span><\/li>
    4. For divorced applicants: Divorce Decree or PSA\u00a0 Marriage Certificate with Annotation on Divorce (<\/span>original + 1 photocopy<\/b>).<\/span><\/span><\/li>
    5. U.S.\/Foreign Naturalization Certificate (<\/span>original + 1 photocopy<\/b>).<\/span><\/span><\/li>
    6. Valid U.S.\/ Foreign Passport (<\/span>original + 1 photocopy of data page<\/b>).<\/span><\/span><\/li>
    7. Applicant\u2019s photos (<\/span>two colored, identical, 2\u2033 x 2\u2033 sized photos with plain white background taken within the past 6 months<\/b>).<\/span><\/span><\/li>
    8. As applicable: Notarized Affidavit of Explanation on How you attained U.S. Citizenship <\/span>(Click here for Affidavit)<\/span><\/a>.<\/span><\/span><\/li>
