Philippine Medical Association (PMA) who shall forward the same to the following professional groups copy furnished DOH.<\/p><p>Visit the PMA website\u00a0<strong><a #1<\/span>\n\t\t\t\t<\/h3>\n\t\t\t\t\t\t\t\t<p class=\"elementor-icon-box-description\">Each member of the foreign medical team must submit the following documents in applying for temporary license to practice during the duration of the mission. All documents must be in English or with English translation:<\/p>\n\t\t\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-530f00e elementor-widget elementor-widget-text-editor\" data-id=\"530f00e\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<div class=\"elementor-text-editor elementor-clearfix\"><ol><li>Valid\/current license from country of origin, authenticated by the Embassy\/Consulate (please refer to Authentication procedures and requirements in the list of Consular Services)<\/li><li>Board certification (specialty), authenticated by the Embassy\/Consulate (please refer to Authentication procedures and requirements in the list of Consular Services)<\/li><li>Curriculum vitae, acknowledged by the Philippine Embassy\/Consulate (please refer to Authentication procedures and requirements in the list of Consular Services)<\/li><li>Passport sized picture in four copies (taken within one year)<\/li><li>Application letter addressed to the President of PMA to contain the following information:<ul><li style=\"list-style-type: none;\"><ul><li>Type of mission (medical, surgical or both)<\/li><li>Duration of the mission and inclusive dates<\/li><\/ul><\/li><\/ul><\/li><li>The following are additional requirements if the foreign mission team members intend to bring drugs, and\/or medicines, medical supplies and medical equipment to be donated or used during the mission:<ul><li style=\"list-style-type: none;\"><ul><li>Itemized list of the items together with the quantity and the expiration dates for drugs and medicines<\/li><li>Written consent from the host local government executive (governor\/mayor) and\/or of the health facility where the mission will be conducted<\/li><li>Names and specialties of the mission team members<\/li><li>Flight details<\/li><li>Deed of donation authenticated at the country of origin (please refer to Authentication procedures and requirements in the list of Consular Services)<\/li><li>Deed of acceptance from the host hospital\/Local Government Unit (LGU)<\/li><li>Drugs and medicines to be donated should have a shelf life of at least 12 months upon arrival in the country and the said medicine shall be used solely for the scheduled mission.<\/li><li>Application must be submitted 60 days before the date of mission to facilitate clearance\/s from DOH and other agencies.<\/li><\/ul><\/li><\/ul><\/li><li>Letter of certification from the sponsoring host organization (LGU or Non-Government Organization (NGO)) certifying the need and type of mission to be conducted. Cost of post-care mission of morbidities\/mortalities arising from the mission will be shouldered as well by the host organization.<\/li><li>Proof of juridical entity (certified true copy of the Securities and Exchange Commission (SEC) registration must be submitted.<\/li><li>Notarized letter of guarantee from both the medical mission group and the host organization assuming full responsibility for the outcome of the missions.<\/li><\/ol><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-20e076c elementor-view-default elementor-vertical-align-top elementor-widget elementor-widget-icon-box\" data-id=\"20e076c\" data-element_type=\"widget\" data-widget_type=\"icon-box.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<div class=\"elementor-icon-box-wrapper\">\n\t\t\t\t\t\t<div class=\"elementor-icon-box-content\">\n\t\t\t\t<h3 class=\"elementor-icon-box-title\">\n\t\t\t\t\t<span >Guideline #2<\/span>\n\t\t\t\t<\/h3>\n\t\t\t\t\t\t\t\t<p class=\"elementor-icon-box-description\">The PMA, upon receipt of all requirements, will forward the application to the concerned specialty societies for evaluation of credentials. The DOH will be informed immediately of the presence of drugs, medical supplies and medical equipment.<\/p>\n\t\t\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-bcc3836 elementor-view-default elementor-vertical-align-top elementor-widget elementor-widget-icon-box\" data-id=\"bcc3836\" data-element_type=\"widget\" data-widget_type=\"icon-box.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<div class=\"elementor-icon-box-wrapper\">\n\t\t\t\t\t\t<div class=\"elementor-icon-box-content\">\n\t\t\t\t<h3 class=\"elementor-icon-box-title\">\n\t\t\t\t\t<span >Guideline #3<\/span>\n\t\t\t\t<\/h3>\n\t\t\t\t\t\t\t\t<p class=\"elementor-icon-box-description\">The DOH shall facilitate issuance of clearance by its FDA and BHDT for the above mentioned goods.<\/p>\n\t\t\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-cb17d17 elementor-view-default elementor-vertical-align-top elementor-widget elementor-widget-icon-box\" data-id=\"cb17d17\" data-element_type=\"widget\" data-widget_type=\"icon-box.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<div class=\"elementor-icon-box-wrapper\">\n\t\t\t\t\t\t<div class=\"elementor-icon-box-content\">\n\t\t\t\t<h3 class=\"elementor-icon-box-title\">\n\t\t\t\t\t<span >Guideline #4<\/span>\n\t\t\t\t<\/h3>\n\t\t\t\t\t\t\t\t<p class=\"elementor-icon-box-description\">After the evaluation of the missioner\u2019s credentials by the concerned Specialty Society, the PMA shall send a letter of endorsement to DOH.<\/p>\n\t\t\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-71bebce elementor-view-default elementor-vertical-align-top elementor-widget elementor-widget-icon-box\" data-id=\"71bebce\" data-element_type=\"widget\" data-widget_type=\"icon-box.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<div class=\"elementor-icon-box-wrapper\">\n\t\t\t\t\t\t<div class=\"elementor-icon-box-content\">\n\t\t\t\t<h3 class=\"elementor-icon-box-title\">\n\t\t\t\t\t<span >Guideline #5<\/span>\n\t\t\t\t<\/h3>\n\t\t\t\t\t\t\t\t<p class=\"elementor-icon-box-description\">The DOH shall then endorse the documents to Professional Regulation Commission (PRC).<\/p>\n\t\t\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-0eaaec3 elementor-view-default elementor-vertical-align-top elementor-widget elementor-widget-icon-box\" data-id=\"0eaaec3\" data-element_type=\"widget\" data-widget_type=\"icon-box.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<div class=\"elementor-icon-box-wrapper\">\n\t\t\t\t\t\t<div class=\"elementor-icon-box-content\">\n\t\t\t\t<h3 class=\"elementor-icon-box-title\">\n\t\t\t\t\t<span >Guideline #6<\/span>\n\t\t\t\t<\/h3>\n\t\t\t\t\t\t\t\t<p class=\"elementor-icon-box-description\">The PRC, after final review, shall issue the Special Temporary Permit to Practice to concerned foreign missioners.<\/p>\n\t\t\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t<div class=\"elementor-column elementor-col-50 elementor-inner-column elementor-element elementor-element-1f634c9\" data-id=\"1f634c9\" data-element_type=\"column\">\n\t\t\t<div class=\"elementor-column-wrap elementor-element-populated\">\n\t\t\t\t\t\t\t<div class=\"elementor-widget-wrap\">\n\t\t\t\t\t\t<div class=\"elementor-element elementor-element-81d74b6 elementor-view-default class=\"elementor-icon-box-title\">\n\t\t\t\t\t<span >Guideline #7<\/span>\n\t\t\t\t<\/h3>\n\t\t\t\t\t\t\t\t<p class=\"elementor-icon-box-description\">The lead time for the processing of the application are as follows:<br>\n\n<br>PMA \u2013 2 weeks before the mission date\n<br>DOH \u2013 1 week before the mission date\n<br>PRC \u2013 3 weeks before the mission date<\/p>\n\t\t\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-21a4981 elementor-view-default elementor-vertical-align-top elementor-widget elementor-widget-icon-box\" data-id=\"21a4981\" data-element_type=\"widget\" data-widget_type=\"icon-box.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<div class=\"elementor-icon-box-wrapper\">\n\t\t\t\t\t\t<div class=\"elementor-icon-box-content\">\n\t\t\t\t<h3 class=\"elementor-icon-box-title\">\n\t\t\t\t\t<span >Guideline #8<\/span>\n\t\t\t\t<\/h3>\n\t\t\t\t\t\t\t\t<p class=\"elementor-icon-box-description\">Post-mission report shall be submitted by the head of the foreign surgical and\/or medical missions, concurred\/noted by the President of the local component society of PMA, and\/or specialty society and the LGU concerned, to the oversight agencies (DOH, PMA, PRC) 15 days after the completion of the mission. All records and audio-visual documents must also be submitted in triplicate form. The post-mission report should contain the following minimum basic information:\n\n<\/p>\n\t\t\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-336fd41 elementor-widget elementor-widget-text-editor\" data-id=\"336fd41\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<div class=\"elementor-text-editor elementor-clearfix\"><ol><li style=\"list-style-type: none;\"><ol><li>No. and age of cases seen<\/li><li>Diagnosis of patients<\/li><li>List of medicines distributed and surgical interventions done<\/li><li>Endorsements to the necessary health facilities (follow-up, further evaluation and management)<\/li><li>Morbidities and mortalities, if any<\/li><\/ol><\/li><\/ol><\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-d863cf5 elementor-view-default elementor-vertical-align-top elementor-widget elementor-widget-icon-box\" data-id=\"d863cf5\" data-element_type=\"widget\" data-widget_type=\"icon-box.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<div class=\"elementor-icon-box-wrapper\">\n\t\t\t\t\t\t<div class=\"elementor-icon-box-content\">\n\t\t\t\t<h3 class=\"elementor-icon-box-title\">\n\t\t\t\t\t<span >Guideline #9<\/span>\n\t\t\t\t<\/h3>\n\t\t\t\t\t\t\t\t<p class=\"elementor-icon-box-description\">All materials submitted to the oversight agencies shall be the property of the Philippine counterpart and may not be used for advertisement, solicitation or medical publication without the written and expressed approval of the local medical society and\/or specialty society of the PMA and the local counterpart.
