notarial services The Consulate, through its Consular Officers, can notarize documents signed by individuals that will be used or presented in the Philippines. The notarized document will have a covering page (“ACKNOWLEDGEMENT”) with an eyelet and a gold seal. Personal appearance of the signatory or signatories is a requirement for consular notarization.  Appointment is required for this service. Click here to book an appointment. CIVIL REGISTRATION Vital or Civil Registration is the recording in the appropriate civil registers the vital acts and events that attest to the civil status of individuals. Documents must be submitted by email first for pre-assessment. Processing of Report of Birth, Marriage & Death are BY MAIL ONLY. Appointment is required for Correction of Error/Change in Name/Supplemental Reporting in Civil Registry documents. What do you want to do?
