The death of a Filipino national should be reported to, and registered with, the Philippine Statistics Authority (PSA) through the Philippine embassy or consulate having jurisdiction over the locality where the event took place.
ONLY deaths that took place in the following states can be registered or reported at the Philippine Consulate General in New York: (1) Connecticut, (2) Delaware, (3) Maine, (4) Massachusetts, (5) New Hampshire, (6) New Jersey, (7) New York, (8) Pennsylvania, (9) Rhode Island, and (10) Vermont.

Consular Mortuary Certificates are issued when the human or cremated remains of a deceased Filipino or non-Filipino are to be transported to the Philippines.
Requirements for shipment of human remains depend upon whether the body has been embalmed or cremated.
Submit application via email
Send one copy each of the documentary requirements listed below to newyorkpcg.atn@dfa.gov.ph.
The Consulate assesses the completeness of the emailed document.
Mail the Civil Registry Application packet
Once emailed documents have been determined to be complete and in order by the Consular Officer, the informant mails to the Consulate the ROD and/or Consular Mortuary Application packet (please see below the contents of the packet), taking note of the envelope’s tracking number. Complete Documentary Requirements
Arrange documents according to the sequence as shown below.
For Report of Death Only
Arrange the four (4)sets of

Payment
Processing Fee: Processing Fee: $25.00 (ROD); $25.00 (Consular Mortuary)
Payment should be in the form of money order or cashier’s check payable to the Philippine Consulate General. Do not send cash by mail. Personal checks are not accepted.
Return Envelope
A self-addressed USPS envelope with tracking number and corresponding amount of stamps, must be included when sending the application. Do not use metered stamps or P.O. Box Return Address.
The Philippine Consulate General assumes no responsibility for any delay or loss in the mail, or while the documents are in the custody of the courier service. The applicant should note the tracking numbers of all envelopes used and submitted.
Ensure that all submitted requirements are correct and in order. Otherwise, incomplete submissions will not be processed and will be returned to the informant at their own expense.
Consulate Processes the Mailed Application
Processing normally takes a maximum of three (3) working days from the date the complete accomplished packet is received by the Consulate.
For Consular Mortuary Application, the documents must be received by the Consulate at least seven (7) working days prior to the departure date.

Using the return envelope provided by the applicant, the Consulate mails back the applicant's processed document and receipt.
Get PSA-authenticated Death Certificate Online
You may request PSA copy of the ROD after 6 to 8 months from the date of reporting at www.psaserbilis.com.ph. To complete the online transaction, you will need the transmittal details (Reference Number, Despatch Number, Despatch Date and Transmittal Date) which can be obtained from the DFA Manila's Consular Records Division with email oca.crd-us@dfa.gov.ph.
Basic Requirements
Report of Death Application Form (Notarized)
Submit four (4) forms typed or printed legibly in black or blue ink. All forms must bear the original signature of the informant AND original notary seal of any US notary public.
Affidavit of Delayed Registration (Notarized)
Required if application is filed more than 1 year after the occurrence.
Must be executed and signed by the informant and duly notarized by any US notary public. Submit four (4) originals signed and notarized.
Valid ID of Informant
Submit four (4) photocopies.
Death Certificate issued by the State Department of Health
Submit one (1) original and three (3) photocopies.
If death certificate does not indicate the cause of death, secure a confidential medical record from the hospital or mortuary (sample can be seen here: https://www.nyc.gov/assets/doh/downloads/pdf/vr/death-certificate-paper-form.pdf)
Certificate of Cremation (for cremated Remains)
Submit one (1) original and three (3) photocopies.
Certificate of Cremation issued by the crematory
Proof of Filipino Citizenship of the Deceased
Submit one (1) original and four (4) photocopies.
Samples of proof of Filipino citizenship are: Philippine Passport, US Permanent Resident Card, Valid Visa or Work Permit; or Dual Citizenship papers (Identification Certificate and Approval Order)
Additional Requirements
(if remains will be shipped to the Philippines)
Burial-Transit Permit (for human remains)
Submit one (1) original/CTC and three (3) photocopies.
Health Certificate (for human remains)
Issued by the State Department of Health stating that the deceased did not die of contagious disease and that the human remains do not pose any danger to public. Submit one (1) original and three (3) photocopies.
Notarized Mortuary Certificate (for human or cremated remains)
Must state that:
(a) the body was properly embalmed/cremated in accordance with internationally accepted practices:
(b) the body/ashes are in sanitary condition for overseas shipment, consistent with existing standards and
(c) the casket/urn has been hermetically sealed and contains only the human remains or ashes of the deceased
Submit one (1) original/CTC and three (3) photocopies of the notarized document
Embalmer's Certificate (for human remains)
Submit one(1) original/CTC and three (3) photocopies
Letter of Acceptance of Remains
Must be duly accomplished and signed. Submit four (4) copies.
Valid ID of the Consignee (if cremated remains, the person who will bring the ashes to the Philippines)
Submit four(4) copies.
Information Sheet
Must be duly accomplished and signed. Submit four (4) copies.
Flight Details
The itinerary must include full flight details and mode of transportation. Submit four (4) copies.
Latest ID of the Deceased
