Copies of Documents must be submitted by email first to newyorkpcg.dual@dfa.gov.ph for assessment.

Processing of applications is by email only.

Once approved, the link to the appointment system will be sent to the applicant via email.

Checklist of Documents required for Dual Citizenship

A. Core requirements for Principal Applicant:

  1. One (1) duly accomplished Dual Citizenship Application Form (click here)
  2. Philippine Statistics Authority (PSA)-issued Birth Certificate (original + 1 photocopy).
  3. Latest Philippine Passport (original + 1 photocopy of data page). 
    1. If the applicant is not able to present it, the Consulate requires a notarized affidavit of loss.
  4. For married women applicants: PSA Marriage Certificate / Report of Marriage (ROM) / US or Foreign Marriage Certificate (original + 1 photocopy). 
  5. For widows: Death Certificate (original + 1 photocopy). 
  6. For divorced applicants: Divorce Decree or PSA  Marriage Certificate with Annotation on Divorce (original + 1 photocopy).
  7. U.S./Foreign Naturalization Certificate (original + 1 photocopy).
  8. Valid U.S./ Foreign Passport (original + 1 photocopy of data page).
  9. Applicant’s photos (two colored, identical, 2″ x 2″ sized photos with plain white background taken within the past 6 months).
  10. As applicable: Notarized Affidavit of Explanation on How you attained U.S. Citizenship (Click here for Affidavit).
  11. U.S. Naturalization Certificate (original + 1 photocopy).
  12. Proof of Payment (to be obtained after the appointment is confirmed through VFS appointment system).

B. Requirements for each dependent child under 18 yrs old:

  1. PSA-issued Birth Certificate (original + 1 photocopy).
  2. Latest Philippine Passport for natural-born Filipinos (original + 1 photocopy).
  3. U.S. /Foreign Certificate of Citizenship (original + 1 photocopy).
  4. U.S. Passport / Foreign Passport (original + 1 photocopy).
  5. Child’s Photos (two colored , identical 2″ x 2″ sized photos with plain white background taken within the past 6 months).
  6. Proof of Payment (to be obtained after the appointment is confirmed through VFS appointment system).
